List of documents for Account Opening


Sr. No Type of Customers Documents/papers to be obtained
1 Individual Accounts (including Walk in / Occasional customers)

1) Copy of the applicable valid identity document

2) “Identity Document” means the following documents for identification of natural persons as applicable:

a) Valid CNIC/ SNIC/ NICOP/ SNICOP for Pakistani citizens;

b) Valid Passport for foreign citizens;

c) Valid POC for persons of Pakistani origin;

d) Valid ARC for registered Aliens in Pakistan;

e) Valid POR Card for Afghan refugees; and

f) Valid Form-B/Juvenile Card for Pakistani citizens who are minors.

3) In case of an individual with shaky/ immature signatures, in addition to CNIC or any of valid document mentioned at Sr. No 1, a passport size photograph of the new account holder besides taking his/her right and left thumb impression on the specimen signature card will be obtained.

4) Visit Report as per point # 4(3 a, b) below, if the business transactions are allowed in personal account where constituent documents are not available

5) Name Screening of individual, NOK and walk in customer is mandatory to be conducted.

2 a) Housewife Accounts

In addition to applicable valid identity documents as per Sr. No.1 above:

a) Obtain a self-declaration for source and beneficial ownership of funds;

b) Update details of funds providers, if any along with customer’s profile; and

c) Identify and verify funds providers if monthly credit turnover is Rs. 5.000 Million and above.

d) Name Screening of Housewife, NOK (if any) and funds provider is mandatory to be conducted.

3 b) Landlords

1) In addition to applicable valid identity documents as per Sr. No.1 above, apply any recommended method for assessment of source of funds/income e.g. Passbook of landholding records etc.

2) Customer Verification Report as per (Annexure-225).

3) Name Screening of Landlord customer and NOK (if any) is mandatory to be conducted.

c c) Farmer / Agriculture

1) In addition to applicable valid identity documents as per Sr. No.1 above, copy of wheat bill or challan / Agri Pass Book / Fard-e-Malkiat / Khasra Gardawari or any other document confirming his status as Farmer/Agriculturist.

2) Customer Verification Report as per (Annexure-225)

3) Name Screening of Farmer / Agriculture customer and NOK (if any) is mandatory to be conducted.

3 Joint Accounts

1) Copy of the applicable valid identity document for each joint account holder

2) In the case of joint accounts, CDD measures on all of the joint account holders shall be performed as if each of them is individual customers of the Bank.

3) Name Screening of all joint account holder and NOK (if any) is mandatory to be conducted.

4 Sole Proprietorships

1) Copy of the applicable valid identity document as per Sr. No. 1 above

2) Any one of the following documents:

a) Registration certificate for registered concerns

b) Sales tax registration or NTN certificate, wherever applicable

c) Certificate or proof of membership of trade bodies etc., wherever applicable

d) Declaration of sole proprietorship on business letter head

e) Account opening requisition on business letter head

3) In order to verify the physical existence of business or self-employed status, branches may conduct physical verification prior to opening of account and keep the report (Annexure-31) along with the customer documents/file. In case of unsatisfactory verification at the time of account re-activation, branch may consider reporting it to CCG and/or may change risk profile, as appropriate.

a) Visit report duly signed by the visiting officer shall be supported by photographs of business place (printed/scanned copy, also signed by the visiting officer) at the time of opening/re-activation of account.

b) In case of rental status of business place/ running business at owned premises/home as a business place, a declaration to this effect shall be obtained while opening/re-activation of account.

4) Name Screening of “Business Name”, individual customer and NOK (if any) is mandatory to be conducted.

5 Small businesses and professions including freelance professionals, self employed individuals/professionals

1) Copy of the applicable valid identity document of the account holder/s as per Sr. No. 1 above

2) Any one of the following documents:

a) Registration certificate for registered concerns

b) Sales tax registration or NTN certificate, wherever applicable

c) Certificate or proof of membership of trade bodies etc., wherever applicable

d) Proof of source of funds/ income

e) Account opening requisition on business letter head.

3) Visit Report as per point # 4(3-a) above

4) Name Screening of “Business Name” and individual customer is mandatory to be conducted.

6 Partnership

1) Copy of the applicable valid identity documents as per Sr. No. 1 above of all the partners and authorized signatories.

2) All of the following documents:

a) Attested copy of ‘Partnership Deed’ duly signed by all partners of the firm.

b) Attested copy of Registration Certificate with Registrar of Firms. In case the partnership is unregistered, this fact shall be clearly mentioned on the Account Opening Form.

c) Visit Report to be made as per point # 4(3-a) above

d) Authority letter, in original, signed by all partners for opening and operating the account ensuring that given mandate does not violate or contradict the provisions of the partnership deed.

e) AOF should contain information about all partners regardless they are signatories or not.

3) Name screening of “entity”, all partners, and authorized signatories is mandatory to be conducted.

7 Limited Liability partnership (LLP)

1) Copy of the applicable valid identity documents as per Sr. No. 1 above of all the partners and authorized signatories.

2) All of the following documents:

a) Certified copies of:

i. ‘Limited Liability Partnership Deed/ Agreement’

ii. LLP-Form-III having detail of partners/designated partner in case of newly incorporated LLP

iii. LLP-Form-V regarding change in partners/ designated partner in case of already incorporated LLP

b) Authority letter signed by all partners, authorizing the person(s) to operate LLP account.

3) Visit Report as per point # 4(3-a) above.

4) Name screening of “entity”, all partners and authorized signatories is mandatory to be conducted.

8 Limited Companies/ Corporations

1) * Copy of the applicable valid identity documents as per Sr. No. 1 above of all directors and authorized signatories.

2) Certified copies of all of the following documents:

a) Resolution of Board of Directors for opening of account specifying the person(s) authorized to open and operate the account. duly signed by the company secretary or those directors as spelled out in Memorandum & Articles of Association.

b) Memorandum and Articles of Association.

c) Certified copy of Latest ‘Form-A/Form-B’.

d) Incorporate Form II in case of newly incorporated company and Form A/ Form C whichever is applicable; and Form 29 in already incorporated companies.

e) For companies registered:

i. under Companies Act 2017, Acknowledgement of Filing is required.

ii. under Companies Ordinance 1984, Certificate of commencement of business is required. ECP registered declaration for commencement of business as required under Companies Act 2017.

f) List(s) of Directors required to be filed under Companies Act 2017, as applicable.

3) Visit Report as per point # 4(3-a) above.

4) Name screening of “entity”, directors, beneficial owners and authorized signatories is mandatory to be conducted. Unwrapping of complex structures must be performed to reach at UBO for perforing KYC and name screening.

*The condition of obtaining photocopies of identity documents of directors of Limited Companies/Corporations is relaxed in case of Government/Semi Government entities, where branches should obtain photocopies of identity documents of only those directors and persons who are authorized to open and operate the account. However, Branches shall validate identity information including C/SNIC numbers of other directors from certified copies of relevant list(s) required to be filed under Companies Act, 2017.

9 Branch Office or Liaison Office of Foreign Companies 1) Copy of the applicable valid identity documents as per Sr. No. 1 above of senior official and/or authorized signatories.

2) All of the following documents:

a) Copy of permission letter from relevant authority i.e. Board of Investment.

b) List of directors on company letter head or prescribed format under relevant laws/regulations.

c) Certified copies of all of the following documents:

i. Form II about particulars of directors, Principal Officer etc. in case of newly registered branch or liaison office of a foreign company

ii. Form III about change in directors, principal officers etc. in already registered foreign companies branch or liaison office of a foreign company

d) Letter from Principal Officer of the entity authorizing the person(s) to open and operate the account.

3) Visit Report as per point # 4(3-a) above

4) Name screening of “entity”, directors, beneficial owners and authorized signatories is mandatory to be conducted. Unwrapping of complex structures must be performed to reach at UBO for performing KYC and name screening.

10 Trust, Clubs, Societies and Associations, etc. 1) Copy of the applicable valid identity documents as per Sr. No. 1 above of:

a) all members of Governing Body/Board of Directors/Trustees/Executive Committee, if it is ultimate governing body

b) all authorized signatories

c) settlor, the trustee(s), the protector (if any), and the beneficiaries

2) Declaration from the Governing Body/Board of Trustees/Executive Committee/sponsors on ultimate control, purpose and source of funds etc.

3) Certified copies of all of the following documents:

a) Certificate of Registration/ Instrument of Trust

b) By-laws/Rules & Regulations

c) Resolution/ Documentation of the Governing Body/ Board of Trustees/ Executive Committee, if it is ultimate governing body, authorizing any person(s) to open and operate the account.

4) List of Trustees duly certified by at least two Trustees should be obtained.

5) Visit Report to be made as per point # 4(3-a) above

6) Name screening of “entity”, members of governing body, major donors and authorized signatories is mandatory to be conducted.

11 NGOs/NPOs/ Charities 1) Photocopy (after original seen) of the applicable identity documents as per Sr. No. 1 above of all members of Governing Body/Board of Directors/Trustees/Executive Committee, if it is ultimate governing body, and authorized signatories.

2) Certified copies of all of the following documents:

a) All relevant Registration documents/Certificate of Incorporation/license issued by SECP, as applicable

b) Memorandum and Articles of Association

c) Incorporation Form II in case of newly incorporated company and Form B-29 in case of already incorporated company

d) Resolution of the Governing Body/ Board of Directors/ Trustees/ Executive Committee, if it is ultimate governing body, for opening of account authorizing the person(s) to operate the account

3) Annual accounts/ financial statements or disclosures in any form, which may help to ascertain the detail of its activities, sources and usage of funds in order to assess the risk profile of the prospective customer

4) Obtain a declaration from Governing Body/Board of Trustees/ Executive Committee/sponsors on ultimate control, purpose and source of funds etc.;

5) Obtain an undertaking from Governing Body/Board of Trustees/Executive Committee/ sponsors to inform the branch about any change of control or ownership during operation of the account; and

6) Obtain a fresh Resolution of the Governing Body/Executive Committee of the entity in case of change in person(s) authorized to operate the account

7) Visit Report to be made as per point # 4(3-a) above

8) Name screening of “entity”, members of governing body, major donors and authorized signatories is mandatory to be conducted.

12 Agents Accounts 1) Copy of the applicable valid identity documents as per Sr. No. 1 above of the agent and principal

2) Certified copy of ‘Power of Attorney’ or ‘Agency Agreement’.

3) The relevant documents/papers from Sr. No. 3 to 11, if agent or the principal is not a natural person.

4) Name screening of “entity”, agent, principal and authorized signatories is mandatory to be conducted.

13 Executors and Administrators 1) Copy of the applicable valid identity documents as per Sr. No. 1 above of the Executor/Administrator

2) Certified Copy of Letter of Administration or Probate

14 Minor Accounts 1) Copy of the applicable valid identity documents as per Sr. No. 1 above of the minor and his/ her parent or natural or Court Appointed Guardian

2) Certified copy of order of appointment of Guardian appointed by Court, if applicable

3) Photocopy of Form-B

4) In case of natural guardian, documentary evidence should be obtained with respect to Guardian’s source of income

5) Name screening of minor and guardian is mandatory to be conducted.

15 Illiterate/ /photo accounts 1) Copy of the applicable valid identity documents as per Sr. No. 1 above

2) Two passport size photographs, one each to be pasted on AOF and SS Card

3) Left and Right Thumb impressions on the AOFs & Specimen Signature Card(SSC)

4) Vernacular Form from the illiterate person

5) Name screening of account holder and NOK (if any) is mandatory to be conducted.

16 Deaf and Dumb Persons 1) Copy of the applicable valid identity documents as per Sr. No. 1 above

2) Medical certificate showing deaf/dumb disability

3) Indemnity bond, format attached, is required from the customer for opening of account

4) Mark such account as “Deaf/Dumb Person Account” on the AOF as well as on SS Card

5) For Illiterate Deaf and Dumb Person, vernacular form will be obtained in addition to the other formalities required from illiterate customers

6) Name screening of account holder and NOK (if any) is mandatory to be conducted.

17 Visionary Impaired persons 1) Copy of the applicable valid identity documents as per Sr. No. 1 above

2) Medical certificate showing Visionary Impaired disability

3) Undertaking, format attached, duly witnessed from the Literate Visually Impaired person

4) Mark such account as Visually Impaired/Blind Person Account” on the AOF as well as on SS Card

5) For Illiterate Visionary Impaired Person, the account opening officer /BOM/BM/DBM shall read out the rules of business/ term and conditions governing the operations of the account in the presence

of a witness known to the concerned illiterate visually impaired/blind person before signing the account opening forms/documents.

6) Thumb impression to be taken on AOF

7) Name screening of account holder and person authorized for operating the account is mandatory to be conducted.

18 Mentally Disordered Person 1) Copy of applicable valid identity documents as per Sr. No. 1 above of mentally disordered person1 and court appointed manager2 under the applicable laws related to mental health.

2) Certified true copy of court order for appointment of manager for mentally disordered person.

3) Verification of identity document through biometric verification from NADRA for both persons i.e. mentally disordered person and the manager appointed by court.

4) Verification of court order from the concerned court.

5) Account would be opened in the name of mentally disordered person and the same will be operated by the court appointed manager.

6) All CDD requirements/formalities should be conducted / completed for both persons.

7) In case of change of manager by the court, the CDD will be conducted for the new appointed manager by the bank afresh.

8) Name screening of account holder and court appointed manager is mandatory to be conducted.

1“Person with Mental Disorder” or ‘mentally disordered person’ means a person with mental illness as defined in the applicable laws on mental health.

2“Court appointed Manager” means a person appointed by the competent court to operate bank account of a mentally disordered person under the applicable laws on mental health.
